SCOPE OF POSITION:

Under the direction of the Health Director, the Program Coordinator is responsible for a wide range of case management activities and programming being offered for persons engaged in treatment for opioid use disorder in the community of Sipekne’katik.  Working within a multi-disciplinary team, the Program Coordinator will act as a liaison between community agencies and the Sipekne’katik Opioid Treatment and Recovery Program, facilitating treatment planning, and delivering targeted interventions across a broad spectrum of health determinants to ensure comprehensive quality of care.  A sound working understanding of community health approaches in First Nations communities and various community-based agencies in Sipekne’katik, and throughout Nova Scotia, will be required.

Responsibilities:

1.     Program Coordination:

a.     Manages program structure and content to ensure clients’ needs are prioritized (includes regular solicitation of feedback from clients).

b.     Facilitates both group and individual programming in a variety of recovery-related domains (social, cultural, spiritual, practical).

c.     Manages program budget.

d.     Organizes collaborative case conferences for individuals who would benefit from a team-based approach to care.

e.     Maintains and organizes standard records in order to facilitate program evaluation and ensure continuity of care.

f.       Participates in program evaluation on an ongoing basis, various tasks may be required, including but not limited to conducting client interviews, consolidating client feedback, and providing program statistics. Formal program evaluation will be done annually.

g.     Assisting in the social elements of hepatitis C and HIV treatment, in collaboration with the on-site medical team and existing provincial support initiatives.

 

2.     Individual Client Support:

a.     Meet with new clients for an orientation session to review programming options, assess the needs of the individual, and review safe practices/harm reduction strategies in the context of treatment for opioid use disorder.

b.     Develop individualized treatment plans with clients.

c.     Supports clients seeking opportunities to make progress on their wellness goals through engaging with various agencies including, but not limited to, education, employment, justice, and family services.

d.     Identifies and mitigates barriers to treatment adherence (for example, organizing transportation).

 

3.     Community Outreach:

a.     Organize and implement community and family education on treatment of opioid use disorder.

b.     Organize stakeholder meetings.

c.     Facilitate collaboration with various community agencies to foster support for individuals in recovery.

 

4.     Dispensing:

a.     Witness dispensing of buprenorphine/naloxone out of the health centre on a part-time basis to ensure ongoing contact with clients, providing opportunity for routine informal check-ins for individuals who do not actively seek out programming and support.

b.     Please note that on site dispensing is currently in the planning stages. We welcome feedback from the selected candidate about how best to incorporate this important function into the Program Coordinator role. Training for witnessed dispensing will be provided.

Other Requirements :

Qualifications:

  • Bachelor’s degree in Social Work, in good standing with the Nova Scotia Association of Social Workers
  • An equivalent combination of experience and training will be considered
  • Related work experience in First Nations Communities will be an asset
  • Strong communication and teamwork skills
  • Certification/experience in addictions support/counselling
  • Vulnerability Sector Check to accompany resume
  • Must have a valid driver’s licence and access to insured & reliable vehicle
Salary Range: Salary is dependent upon qualifications and experience.
